Scheduled appointments are available on the hour Monday, Wednesday, Friday, 8am-6pm (last appointment 5pm) and Tuesday, Thursday 2pm-6pm (last appointment 5pm). Saturday hours can be scheduled on an as needed basis directly with Ted at the Clinic (503) 517-8996.

For the first visit, please be 10 minutes early in order to fill out the intake information, or download the form from our Forms page and fill it out ahead of time. Parking is on the street and usually available on the same block as Back in Action Physical Therapy. Take care to park where there is unlimited time for parking.

If you are using insurance for payment, you will need to bring your insurance card to the first appointment.

The Physical Therapy profession has direct access in the state of Oregon. This means as a client you are able to schedule yourself directly without a referral from a primary care practitioner. I have listed below how this works for private pay vs billing insurance for payment.

Private Pay: When paying with cash, check, or credit/debit card for physical therapy services, you can schedule for physical therapy and be seen for treatment for up to 30 days without contacting your primary care practitioner. After 30 days we need the primary care provider (PCP) to certify (sign) the treatment plan or have you follow-up with your PCP. Health Insurance billing: When billing health insurance, you can schedule yourself for physical therapy without a referral. However, I will need to send your initial assessment to your primary care provider to get a signature. Some insurance plans do require a referral, although in most cases the insurance company will accept the signed treatment plan that I send to your PCP as a referral.

If Back in Action is out of network for your private health insurance, you can still be seen and pay your regular in-network co-payment or deductible.
